EDUCATIONAL CASE MANAGER -FT (GRANT FUNDED)

DALEY COLLEGE

City Colleges of Chicago (CCC), the largest community college system in Illinois, is made up of seven colleges and five satellite sites, providing more than 70,000 students each year access to a quality, affordable education.

Richard J. Daley college, as a co-grantee with Anixter Center, a community based organization, has a two-year grant to fund an innovative and inclusive post-secondary initiative for young adults with intellectual and developmental disabilities who have aged out of the CPS school system. The grant is part of the inclusive Pathways: After 22 Program that strives to expand the options that families and young adults have for higher education, life skills and job training, employment, and leadership growth.

Staff from Daley College in partnership with Anixter and Special Olympics Chicago/Special Children’s Charities (SO/SCC), will develop and implement a pilot program designed to provide students with intellectual and development disabilities with intense support in job skill development to prepare them to join the workforce, offering them academic and occupational course work and a rich cultural and social college experience. Daley College will also create a Career Advancement basic certificate program designed specifically for students with disabilities. 

PRIMARY OBJECTIVE

Reporting to the Program Director- Comprehensive Transition Program (CTP), the Educational Case Manager interacts directly with prospective and enrolled students to assist them as they pursue their goals in learning job development skills, experiencing campus life and participating in campus activities, and participating in the Comprehensive Transition Program (CTP)/Occupational, Life and Academic Sills (OLAS) Program to earn a Career Advancement certificate program.

An Educational Case Manager works with participants to assess their academic goals, job aspirations and needs related to attaining employment and a career pathway. Performs related duties as required.

 ESSENTIAL DUTIES

  • Meets individually with candidates to obtain information, assess their barriers encountered in meeting goals and assess their academic goals, job aspirations and needs related to attaining employment and a career pathway.
  • Conducts individual student assessment and connects students with college resources and support services to include advising, ACCESS Center, and Wellness Center, as well as social and support services from external agencies.  
  • Coordinate appropriate curricular resources to ensure student’s objectives are met in an organized and sequential manner.
  • Conducts regular follow-up assessments of participants to monitor progress on interim steps and objectives, as well as identifies barriers encountered, ensuring all student meet their goals.
  • Links participants with appropriate educational interventions. Provides referrals for appropriate education and professional resources as needed.
  • Documents all interactions in the After 22 (College to Career Advancement Certificate) Initiative database as well as maintains files for all participants in the caseload.
  • Maintains confidentiality standards for all information, sharing only with the After 22 (College to Career Advancement Certificate) Director and Chicago After 22 (College to Career Advancement Certificate) staff.
  • Develops written and verbal reports for the After 22 (College to Career Advancement Certificate) Director as requested.
  • Delivers presentations to community organizations about the After 22 (College to Career Advancement Certificate)’s target population and services offered. 
  • Participates in ongoing After 22 (College to Career Advancement Certificate) Initiative conference calls with peers from other After 22 (College to Career Advancement Certificate) locations and maintains personal networks.
  • Actively participates in Chicago After 22 (College to Career Advancement Certificate) fundraising activities.
  • Attend in-person meetings as scheduled.
  • Performs other duties as assigned.

QUALIFICATIONS

  • A Bachelor’s degree from an accredited college or university in Social Work, Education, or a related field.
  • Two years of experience in a role related to advising, counseling, or providing career guidance to students with disabilities or adult learners
  • Demonstrated knowledge and understanding of challenges facing students with disabilities
  • Bilingual/English/Spanish preferred
  • Excellent written and verbal communication skills
  • Strong interpersonal and presentation skills
  • Ability to work within a culturally diverse environment 
  • Proficiency in the use of Microsoft Office (Word, Excel, PowerPoint)
