CCC has a Comprehensive Transition Program (CTP) for students with intellectual disabilities called After 22. CTP/After 22 offers both credit and non-credit opportunities for students with intellectual disabilities after they graduate or leave CPS at the age of 22.

After 22 is part of the District Office’s Accessibility Services, which also includes the ACCESS Centers, Interpreter Services, and High School Transitions.

PRIMARY OBJECTIVE

Reporting to the Exec Dir-Access & Retention, the Workforce Success Coach-Comprehensive Transition Program (DO/Grant Funded) supports students enrolled in CCC’s Comprehensive Transition Program (CTP) by providing individualized career coaching, job readiness training, and employment placement assistance. This role ensures that students with intellectual and developmental disabilities (IDD) have equitable access to workforce opportunities and collaborates with internal and external partners to create inclusive, sustainable career pathways. Performs other duties as assigned.

This position is grant funded through at least September 30, 2030.

ESSENTIAL DUTIES

Provides students with individualized and group career coaching, job readiness training, and ongoing support, including interviewing and workplace skills.  

Develops comprehensive Individual Career Success Plans and employment strategies aligned with students’ strengths and goals.  

Coordinates internships, job shadowing, and work-based learning experiences to build practical skills and career exposure.

Collaborates with employers, college teams, ACCESS Centers, and community organizations to create inclusive job opportunities, advocate for workplace accommodations, and support holistic student success.  

Builds and maintains partnerships with community-based organizations and social service agencies to address student needs such as mental health, housing, transportation, financial well-being, and other wraparound supports.

Conducts needs assessments to identify gaps in services, inform partnership development, and make recommendations to leadership for improving program effectiveness.  

Assists in internal processes to assess and strengthen employer and community partnerships, and coordinates mentoring, workshops, and professional development activities for students.

Manages case records and service plans for students requiring additional support, facilitating timely follow-up and continuity of services.  

Documents interactions and referrals in the database system according to program and compliance requirements.  

Tracks employment outcomes and prepare reports for grant compliance, including comparative analysis over time and by program.

Assists with screening, intake, orientation, and assessment of prospective program participants.

Performs other duties as assigned.

QUALIFICATIONS

Bachelor’s degree from an accredited college or university in Special Education, Rehabilitation Counseling, Workforce Development, Human Services, or related field.

Minimum of two years of experience supporting individuals with intellectual and developmental disabilities (IDD) in employment, transition, or postsecondary education programs.

Knowledge of ADA, Section 504, and best practices for inclusive employment, universal design, and workplace accommodations.

Ability to work collaboratively and effectively with students, faculty, staff, employers, and community partners at all levels

Excellent interpersonal, written and verbal communication skills

Excellent organizational skills

Ability to work with and relate to a culturally diverse student body, faculty and staff  

Ability to work independently and as part of a team  

Strong advocacy and relationship-building skills, with the ability to engage employers and community partners.

Proficiency in Microsoft Office 365 (Word, Excel, PowerPoint, Outlook, Teams) and virtual meeting platforms.

Offered salary will be determined by the respective collective bargaining agreement and applicant's education, experience, knowledge, skills and abilities.
