The After 22 Program is a Comprehensive Transition and Post-Secondary (CTP) Program designed to provide students with intellectual and development disabilities with support in job skill development to prepare them to join the workforce, offering them academic and occupational course work and a rich cultural and social college experience. Students will receive a Career Advancement Certificate upon completion of the program.

PRIMARY OBJECTIVE

Reporting to the DO Director of Comprehensive Transition Programs (CTP), with a dotted line to an assigned College administrator, the Coordinator-Comprehensive Transition Programs (After 22) is responsible for assisting the DO Director in administering the day to day operations of the After 22 program on campus, creating and developing Individual Pathway Plans (IPPs) with each enrolled student, and coordinating and ensuring the provision of supportive services to enrolled students in the After 22 program. Performs related duties as required. 

ESSENTIAL DUTIES

·         Reviews disability-related documentation, program application, and other supporting documents to assist in the intake and interview process for prospective After 22 students.

·         Coordinates and conducts new student orientation sessions for admitted After 22 students and families, ensuring they understand the program, expectations, and available resources.

·         Creates and regularly updates Individual Pathway Plans (IPPs) with each enrolled student, conducting follow-up assessments to track progress toward interim goals and objectives. Identifies and addresses barriers to student success, ensuring all students are on track to meet their goals.

·         Assists students in accessing a wide range of college resources and support services, including Advising, the ACCESS Center, the Wellness Center, and external social and support services, ensuring comprehensive support for each student.

·         Establishes and maintains direct communication with faculty members regarding program participants, ensuring they are informed about student needs and progress.

·         Tracks and maintains accurate records for all participants in the caseload, updating the student database at least monthly to ensure all files are current and complete.

·         Provides regular updates to the District Director, including weekly meetings and written reports, to communicate relevant information about program participants, progress, and needs

·         Coordinates appropriate curricular resources to support student in meeting their academic and personal objectives in a structured, sequential manner.

·         Upholds strict confidentiality standards for all student information, sharing only with authorized individuals such as the District Director, After 22 staff, ACCESS Staff, and others directly involved in the program as needed.

·         Participates in ongoing program meetings with stakeholders and builds and maintains professional networks to enhance the program's effectiveness.

·         Maintains awareness of and shares best practices for students with disabilities in higher education.

·         Performs other duties as assigned.

QUALIFICATIONS

·         A Bachelor’s degree from an accredited college or university in Education, Rehabilitation Counseling, Psychology, or a related field.

·         Two years of professional work experience working directly with students with Intellectual/Developmental Disabilities.

·         A combination of education and experience may be taken into consideration based on the applicant’s credentials.

·         Good knowledge of requirements of Title II of the Americans with Disabilities Act (ADA) and Section 504 of the Federal Rehabilitation Act of 1973 as it relates to reasonable accommodations for students with disabilities in a higher education setting.

·         Experience in reading and interpreting Individualized Education Plans (IEPs), Psychological Evaluations, and related disability documentation.

·         Up-to-date knowledge of relevant adaptive technologies and equipment.

·         Knowledge of functional limitations relating to Intellectual and Developmental disabilities and related conditions.

·         Knowledge of support services that address the unique needs of students with Intellectual and Developmental Disabilities and related conditions.

·         Excellent interpersonal and communication skills

·         Strong organizational and problem-solving skills

·         Ability to work with and relate to a culturally diverse student body, faculty, and staff

·         Ability to maintain positive working relationships with students, faculty, and colleagues.  

·         Ability to exercise tact and diplomacy in handling sensitive situations

·         Proficient in the use of Microsoft Office 365 (Word, Excel, PowerPoint, Outlook) with the ability to integrate technology into daily work processes.
